If you are unsure if your appointment reason will require a scheduled appointment, please contact the clinic, as we will be more than happy to assist you.
Walk in patients are fitted in between scheduled appointments, so you should be prepared to wait.
Common wait times for walk in appointments can range from 10- 40 minutes.
All of the Staff at the Alpine Medical Clinic do their best to keep appointments on schedule, however in order for the physicians to give the best and most appropriate treatment; we can sometimes be delayed due to circumstances beyond our control. We appreciate your patience, however if you are unable to wait, please notify the front desk and we will be happy to reschedule your appointment.
Transfer of Medical Records
Whether you are joining, or leaving the Alpine Medical Clinic due to a move, your new physician may require your medical chart. Your medical chart allows a physician to continue treatment by picking up where the last physician left off. It can also be an important tool for the physician to understand previous illnesses. In any case, if you are wishing to make a Request for Transfer of Records to either have records sent to the Alpine Medical Clinic or from, this request must be made in writing.
The request for transfer of medical records is an uninsured service, and therefore not covered by Canadian Health Care. Please beware there might be a fee associated which is at the sending doctors discretion and is the responsibility of the patient.
The regulation requires that physicians keep medical records for a certain period of time. For adult patients, the rule is that records must be retained for 10 years from the date of the last entry in the record. For patients who are children, the regulation requires that the physician keep the record until 10 years after the day on which the patient reached or would have reached the age of 18 years. The fees for transferring the chart are for the proper storage, copying, and faxing or sending of the chart.
If you wish to have your chart transferred to, or from the Alpine Medical Clinic, please see the links below for the Request for Transfer of Records form which you may print off and fax to the clinic.
Previous charts are not always necessary for treatment, speak to your physician and decide if the chart is needed.
